COVID-19 Vaccine Information

Vaccine is now available by appointment on Tuesdays and Thursdays through the Health Department, or offered to homebound residents by speaking with a member of our front desk. To schedule an appointment please call (815) 941-3404. Residents may also choose to visit vaccine clinics in another county or a local pharmacy.

In response to the new booster authorization, the Grundy County Health Department is pleased to announce the following walk-in COVID vaccination clinics for residents 12 years and older.

Friday, June 17
10:00am-1:00pm 
Grundy County Administration Building, 1320 Union Street, Morris 

All 3 vaccines will be available at each clinic, as supplies last. Doses to be administered include 1st, 2nd, 3rd, and boosters. Unless obtaining a 1st does, vaccine record of previous doses administered must be presented.

The Illinois Department of Public Health (IDPH) is launching a new immunization portal, Vax Verify, that will allow Illinois residents 18 years and older to check their COVID-19 vaccination record.

Vax Verify can be accessed at  https://idphportal.illinois.gov.

Update: May 12 – The FDA has approved the Emergency Use Authorization (EUA) for Pfizer/BioNtech vaccine. In accordance with (410 ILCS 210/) Consent by Minors to Health Care Services Act, this form allows a parent/legal guardian of a minor (14 years and older) to authorize vaccine administration and designate an adult to accompany the minor.  Minors under 14 years of age must be accompanied by a parent/legal guardian
