COVID-19 Vaccine Information

 

The Health Dept has Pfizer, Moderna and Novavax vaccines in stock, including the new bivalent from Pfizer and Moderna.

Vaccine is offered by appointment by calling 815-941-3113.

You may also visit vaccines.gov to find another vaccine provider near you. 

The Health Depart does not offer COVID testing. Please check with your doctor’s office, a local immediate care center or a local pharmacy for testing protocol.

The Illinois Department of Public Health (IDPH) is launching a new immunization portal, Vax Verify, that will allow Illinois residents 18 years and older to check their COVID-19 vaccination record.

Update: May 12 – The FDA has approved the Emergency Use Authorization (EUA) for Pfizer/BioNtech vaccine. In accordance with (410 ILCS 210/) Consent by Minors to Health Care Services Act, this form allows a parent/legal guardian of a minor (14 years and older) to authorize vaccine administration and designate an adult to accompany the minor.  Minors under 14 years of age must be accompanied by a parent/legal guardian
